I have fixed this situation a few times. If there is not an obvious gap or hole, it usually takes opening up all the materials and systems surrounding and meeting up with the threshold. This might be:

the existing flashings and sealings

the nearby wall siding

the membrane roof

and sometimes even removing the threshold itself

Once you can see what is going on under, in, and around there, then everything can be put back together keeping the water out.

You should be able to use 50mm coolag with vapour barrier then 3g felt and glass fiber under lay finished with torch on mineral finish last at leat 25 years depends on heights on ur up stands on roof with thickness of insulation .then cost more to start to raise facia boards at front so water goes of end of roof.otherwise consult any roofing contractors in your area.
